Residents may receive visitors 7 days without appointment between 1pm-7pm. Until further notice, NYSDOH requires valid proof of COVID-19 testing prior to visitation. Two days prior for PCR test or one day for Rapid test.

WOUND CARE PROGRAM

Central Island Healthcare’s Comprehensive Wound Care Program is designed to provide the latest medical treatments specific to the individual patient with care and compassion.
Wound Care Program
After a comprehensive assessment of the history and current status of the patient’s wound, the physicians on our Wound Care Team develop an individualized treatment plan. Throughout the treatment program, specially trained wound care nurses implement treatments, while our physicians carefully monitor the progress. Our patients are informed during every phase of the treatment plant, making sure that he or she understands the treatment and the reasons behind it.

The Central Island Professional Wound Care Team includes our in-house Medical Director, a wound care specialist, and our specially-trained wound care nurses working together with physical therapists, occupational therapists, dietitians and our pharmacy ensuring a continuous quality treatment plan.

For the convenience of our patients, our Wound Care Team performs multiple bedside treatments. For patients requiring outpatient treatment, Central Island Healthcare has relationships with local Wound Care Centers to ensure continuum of care for the best patient outcomes.

The Wound Care Team

Dr. Rai
Dr. Rai is a specialist in the field of Physical Medicine and Rehabilitation. He focuses on the health and well-being of the whole person, utilizing the body’s natural tendency towards health and repair to achieve higher levels of function and independence. He believes in the importance of strength and physical conditioning to maintain optimal health, and to treat and prevent injury. He emphasizes that nutrition plays a vital role in the pursuit of a healthy body. Dr. Rai treats patients as partners in the healthcare dynamic, and he practices a teaching model of medicine to help patients achieve their desired health and lifestyle goals. His areas of expertise include pain management, sports medicine and the treatment of various disabilities. He is an expert in the field of rehabilitation following limb loss.
Dr. Moshe Shirazi
A graduate of the Sackler School of Medicine and certified from the American Board of Internal Medicine, Dr. Shirazi brings over fifteen years of talent to Central Island Healthcare. As a member of the American College of Physicians, the American Medical Association, and the New York State Society of Internal Medicine, Dr. Shirazi has a core concentration in internal medicine. As Medical Director of Central Island Healthcare, he oversees both the attending physicians and our nursing staff to ensure that the proper plan of care is being implemented and followed which will in turn optimize wound care prevention and management.
Jeanette Blauvelt, RN
Working at Central Island Healthcare for over 20 years, Jeanette has worked her way from medication nurse, to head nurse, to supervisor and is now focusing her career on wound care. Her experience includes time spent as an emergency room nurse in an acute care setting. Jeanette graduated from Molloy College with a Bachelor’s of Science in Nursing in 1997, after obtaining her associates degree in Nursing from Nassau Community College in 1995. As Central Island’s Wound Care Nurse, Jeanette acts as the primary liaison between doctors and our nurses. Jeanette ensures that the clinical knowledge of our nurses remains at a superior level in regards to the prevention, treatment and healing of wounds.
